Pre-Authorization & Verification Process
All insurance claims and cashless admissions are subject to verification prior to admission. Pre-authorization processing times vary based on the specific Third Party Administrator (TPA) and insurance provider. Patients are advised to submit policy details at least 48 hours prior to planned elective surgeries.
Documents Required for Cashless Claims
To file a pre-authorization request for surgical urology treatment (e.g. laser stone removal or prostate surgery), please provide:
- Active health insurance policy copy or e-card.
- Doctor's consult prescription recommending surgical admission.
- Diagnostic reports (e.g. Ultrasound, CT scans showing stone size or prostate dimensions).
- Aadhaar card and PAN card of the patient.
- Completed pre-authorization form (available at our reception desk).
